I understand that participation in cheerleading involves physical activity including tumbling , jumping, stretching ,stunting ,and conditioning.

I acknowledge that injury can occur during participation and voluntarily allow my child to participate in Believe Cheer activities.

I release Believe Cheer, its coaches, staff, and practice facilities from liability that may occur during particiation, except in cases of gross negligence.

I certify that my child is physically able to participate and that I will notify staff of any medical conditions or concerns.

PHOTO/VIDEO RELEASE AUTHORIZATION

I herby grant permission for Believe Cheer to photograph and /or video record my child during practices, events, performances, competitions, camps , and program activities.

I understand that these images and videos may be used for promotional purposes including, but not limited to :

• Social Media

• Website Content

• Flyers and marketing materials

• Team Promotions

• Program Advertising

I understand that no compensation will be provided for the use of these images or videos.

I also understand that Believe Cheer will use discretion and professionalism when sharing images related to the program.

Payment Agreement

I understand that registration is not compete until payment has been submitted.

Registration fee and tuition payments secure my athlete's place in the program.

Payment may be submitted through Zelle or through the Believe Cheer website payment center.

I understand that registration fees are non-refundable unless otherwise stated by Believe Cheer.
